The Healthy Steps Patient/Program Coordinator will be part of a dynamic pediatric team within the Brookdale Family Care Center (BFCC).  The Coordinator will work directly with the Project Director and Healthy Steps Specialist/Assistant Project Director and the pediatricians to provide administrative support for all aspects of this new program and will function as the primary interface between the patient and the providers.  This is a unique opportunity to be part of the start-up team of this new model of integrated health care at Brookdale.  The ideal candidate will have excellent interpersonal skills, be able to work as part of a team, and be flexible and adaptable as the program takes shape.   We are also seeking a candidate who truly enjoys working with and interacting with families and striving to meet patient’s needs.  5 days/week (FT)

Responsibilities:

 Under the direction of the Project Director and Healthy Steps Specialist/Ass’t Project Director, assists with the implementation of Healthy Steps program and performs necessary support duties.
• Assist in the development of a new office workflow that will accommodate the Healthy Steps program within the clinic; be willing to troubleshoot and problem solve around this issue.
• Work with clinic staff and other departments and outside agencies to help recruit new families.
• Responsible for scheduling all Healthy Steps Specialist appointments for families.
• Greet patients via telephone, enter and update their demographics in the practice management system.
• Serves as a source of information about the Healthy Steps project.
• May act as liaison with related programs or projects of other institutes or with government agencies, keeping Project Director and specialist/Ass’t Director informed of relevant information.
• Manage all paperwork/screening forms for patients
• Assist with data entry and data collection; may assist on research/evaluation projects
• Develop effective filing systems and maintain all files and records
• Coordinate creation & distribution of patient education brochures
• Triage and manage more complex telephone calls
• Maintain confidentiality and privacy consistent with HIPAA guidelines.
• Assist with coordination and development of parenting groups
• May assist on preparation of grant proposals
• Works on special projects as directed, such as coordination of presentations
 

Qualifications:

 College degree preferred, or an equivalent of education plus relevant experience in the health care environment. 

Skills & Competencies
• Effective customer service and interpersonal skills
• Reliable with patience and professionalism
• Strong proficiency in keyboard skills and knowledge of MS Word, Outlook and Excel preferred
• Good command of the English language.
• Flexible and able to work in a high-pressure team environment while multi-tasking
• Able to work independently AND in a team environment.
• Willingness/ability to learn hospital’s Electronic Medical Records System (EPIC)
• Willingness/ability to learn hospital billing procedures around HMO, Managed Care, Medicaid, & other Third-Party Payers. 
• Organized with the ability to prioritize and multi-task
• Able to respect the strict confidentiality of medical records and information.
