Fingerprints are required for:

• Family Day Care Providers
• Group Family Day Care providers
• Group Family Day Care Assistants
• Family members –18 years and older

Fingerprints are taken at:

Bring:

• Photo ID (driver’s license, passport, etc)
• The Booklet ID # (Located inside the application booklet, at the bottom left hand side on the back of each page)

Fingerprinting Information For NYS Teacher Certification and Teaching in a NYC Public School

For NYS Teacher Certification Candidates:

• Fingerprints may be taken at:
  • The NYS college/university you are attending (if fingerprinting is offered)
  • A Board of Cooperative Educational Services (BOCES)
  • A local police station in New York State
  • Or at the:
    • NYC Department of Education
      Division of Human Resources
      Employee Information Center

For NYS Certified Teachers planning to teach in New York City Public Schools:

• If you have been fingerprinted in New York State for your NYS Teacher Certification those fingerprint results can be used to meet the NYC Department of Education fingerprint requirement.
• Complete an OSPRA-103 form. Download It
  • This form allows the NYS Education Department to release the fingerprint results they have received from other NYS school districts or designated fingerprint facilities to the NYC Department of Education.
  • Send your completed OSPRA-103 form to:

• The processing fee of $115 will be collected through an automatic payroll deduction when full-time employment with the NYC Department of Education begins.

For Non-NYS Certified Teachers, planning to teach in NYC Public Schools:

• Get a ‘nomination’ from a principal
• Go to:

  • 2 forms of ID
    • Photo ID (Driver’s License) and Social Security Card or
    • Photo ID (i.e. Driver’s License) and original birth certificate or
    • Valid U.S. passport (which serves as both forms of ID
